How do I become a Philhealth member & renew membership?

If in the Philippines, submit the following documents to any PhilHealth office:

1. Properly accomplished duplicate copies of the PhilHealth Member Registration Form (PMRF), if initial registration, or renewal form if renewal of membership;
2. Attach supporting documents for declared legal dependents, if declaring dependents;
3. Any proof of being an active OFW and;
4. Pay the corresponding premium contribution of P900 to any PhilHealth office

Meanwhile, if you are abroad:

1. Submit a properly accomplished PhilHealth Premium Payment Slip (PPPS);
2. Submit duplicate copies of the PhilHealth Member Registration Form (for initial registration only);
3. Attach any of the following documents:
• Birth Certificate;
• Baptismal Certificate;
• POEA ECARD/SSS/Company ID
• Passport
• Any other valid ID / document acceptable to the Corporation;
4. Pay the corresponding premium contribution of P900 (or its equivalent in the local currency of the host country) to any accredited collecting agent.
