{"id":15574,"date":"2016-09-12T01:15:20","date_gmt":"2016-09-12T01:15:20","guid":{"rendered":"http:\/\/www.pinoy-ofw.com\/directory\/?p=15574"},"modified":"2016-09-12T01:15:20","modified_gmt":"2016-09-12T01:15:20","slug":"what-documents-are-needed-in-filing-for-sss-retirement-benefit","status":"publish","type":"post","link":"https:\/\/www.pinoy-ofw.com\/directory\/what-documents-are-needed-in-filing-for-sss-retirement-benefit-15574.html","title":{"rendered":"What Documents Are Needed In Filing For SSS Retirement Benefit?"},"content":{"rendered":"<p>SSS Member Question: What documents should I prepare to file for SSS retirement benefit?<\/p>\n<p>Before proceeding to applying for retirement benefit, it would be prudent to review the eligibility of a member to receive his or her SSS retirement benefit.<\/p>\n<p><strong>Eligibility<\/strong><br \/>\nA member is qualified to avail of this benefit if:<\/p>\n<ol>\n<li>Member is 60 years old, separated from employment or ceased to be self-employed, and has paid at least 120 monthly contributions prior to the semester of retirement.<\/li>\n<li>Member is 65 years old, whether employed or not, and has paid at least 120 monthly contributions prior to the semester of retirement.<\/li>\n<li><span style=\"color: #003366\">Underground Mineworkers:<\/span>\n<ul>\n<li>Aged 55 years old and is an underground mineworker for at least 5 years (either continuous or accumulated) prior to the semester of retirement but whose actual date of retirement is not earlier than March 13, 1998; separated from employment or has ceased self-employment; and has paid at least 120 monthly contributions prior to the semester of retirement.<\/li>\n<li>Aged 60 years old, whether employed or not, and has paid at least 120 monthly contributions prior to the semester of retirement.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><strong>Required documents<\/strong><\/p>\n<ul>\n<ul>\n<li><a href=\"http:\/\/wwi.sss.gov.ph\/sss\/DownloadContent?fileName=SSSForm_Retirement_Claim.pdf\" target=\"_blank\" rel=\"noopener\">Retirement Claim Application<\/a><\/li>\n<li><a href=\"http:\/\/wwi.sss.gov.ph\/sss\/DownloadContent?fileName=SSSForm_Specimen_Signature_Card.pdf\" target=\"_blank\" rel=\"noopener\">DDR Signature Card<\/a><\/li>\n<li>DDR Savings Account form;<\/li>\n<li>Certificate of Separation from last employer (if member is less than 65 years old) ;<\/li>\n<li>Passbook or ATM Card (if pension);<span style=\"color: #003366\"><em>A member must open a single savings account in the bank of his choice and submit to the SSS his savings account number and a photocopy of his passbook or ATM Card upon filing of his application. The original copy of the passbook must be presented for authentication purposes. <\/em><\/span><\/li>\n<li>Certified true copies of birth or baptismal certificate of dependent children;<\/li>\n<li>Certified of cessation of business or practice of profession (for self-employed members who are less than 65 years old);<\/li>\n<li>Certified true copy of marriage certificate<\/li>\n<li>Proofs of filiation for illegitimate dependent children<\/li>\n<li>UMID or SSS biometrics ID card or two (2) other valid IDs, both with signature and at least one (1) with photo and date of birth.<\/li>\n<li><em>* Other supporting documents may be requested by SSS depending on the evaluation of the claim<\/em><\/li>\n<\/ul>\n<\/ul>\n<p><strong>Filing Procedure<\/strong><br \/>\nApplications for retirement benefit are filed at any SSS branch or representative office.<\/p>\n<p><a href=\"http:\/\/www.pinoy-ofw.com\/directory\/sss-locations-in-the-philippines\"><strong>SSS locations in the Philippines<\/strong><\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>SSS Member Question: What documents should I prepare to file for SSS retirement benefit?
