Requirements<\/strong><\/p>\n

    \n
  1. Original and photocopy of passport (must be valid about six months before date of travel)<\/li>\n
  2. Original and photocopy of visa<\/li>\n
  3. One 2\u00d72 or passport-size ID<\/li>\n
  4. One valid ID with photograph (eg., SSS ID, GSIS E-card, PRC ID, driver\u2019s license, postal ID, ARC, etc.)<\/li>\n
  5. Photocopy of Immigrant Data Summary for USA-bound emigrant (must not be detached from visa packet) except for K visa holder<\/li>\n
  6. Original and photocopy of Confirmation of Permanent Residence for Canada-bound emigrant (must not be torn or signed before departure)<\/li>\n
  7. Original and photocopy of Certificate of Eligibility for Japan-bound emigrant<\/li>\n
  8. Original and photocopy of Nulla Osta (for Italy-bound emigrants)<\/li>\n
  9. Original and photocopy of letter of approval for Work to Residence visa (for New Zealand-bound emigrants)<\/li>\n
  10. Printed copy of Visa Grant Notification Letter (for Australia-bound emigrants)<\/li>\n
  11. Photocopy of employment contract (for immigrant workers)<\/li>\n
  12. Duly completed registration form for Emigrants (click here) or for Immigrant Workers (click here)<\/li>\n
  13. Payment of P400.00 registration fee<\/li>\n
  14. Attendance in the PDOS, guidance counseling or peer counseling session<\/li>\n<\/ol>\n

    For youth emigrants aged 13 to 19, the following additional documents are also required:<\/b><\/p>\n

      \n
    1. Photocopy of parent\u2019s passport or valid identification card, if accompanied by parent\/s during registration<\/li>\n
    2. Letter of authorization from parent\/s authorizing the guardian to register the child with CFO on their behalf (if accompanied by guardian during registration)<\/li>\n
    3. Original and photocopy of DSWD travel clearance (click here for the new guidelines on issuance of travel clearance<\/a>)<\/li>\n<\/ol>\n

      Exemptions<\/strong><\/p>\n

      For children 12 years old and below are exempted from attending the PDOS. They must, however, be registered, even if by proxy. The following additional documents are required if the proxy is not the legal\/natural\/adoptive parent of the registrant.<\/p>\n

        \n
      1. Original and photocopy of one valid ID of the proxy<\/li>\n
      2. Special Power of Attorney authenticated by Diplomatic Post if parent\/s\/ or guardian\/s are based overseas; or a duly notarized authorization letter executed by parent\/s or guardian\/s while in the Philippines.<\/li>\n
      3. Other documents that may be required by the registration officers.<\/li>\n<\/ol>\n

        The following emigrants are exempted from attending the PDOS and registering in person with the CFO, provided that they are registered by a proxy:<\/p>\n

          \n
        • Those who are 60 years old and above;<\/li>\n
        • Those incapacitated due to permanent or long-term ailments; or<\/li>\n
        • Those with mental health issues or psychologically-challenged.<\/li>\n<\/ul>\n

          In the registration of the above emigrants, the proxy is required to present the following documents in addition to the usual required documents:<\/p>\n

            \n
          • Medical certificate\/report (except those 60 years old and above);<\/li>\n
          • Letter of authorization from the emigrant (if applicable to the case); and<\/li>\n
          • Original and photocopy of a valid identification card with photograph of the proxy.<\/li>\n<\/ul>\n

            Note: After satisfactory completion of the requirements, an emigrant registration sticker is affixed to the emigrant\u2019s passport<\/i><\/p>\n
