OWWA Scholarship: OFW Dependent Scholarship Program

Requirements:

1. Filled out the application form
2. 1×1 ID photo
3. For land-based OFWs:
   • Photocopy of
     • Name and photo
     • Latest visa stamp
     • Latest departure and arrival stamp
4. For sea-based OFWs:
   • Photocopy of
     • Name and photo
     • Latest embarkation and disembarkation stamp
5. Proof of relationship to OFW
   • If child:
     • PSA copy of the birth certificate of the applicant
   • If sibling:
     • PSA copy of the birth certificate of both OFW and applicant
     • CENOMAR of OFW
6. Transcript of records (original or certified true copy)
7. Certificate of good moral character
8. Barangay certificate
9. Medical certificate
