It should be noted however that NBI Form No. 5 has a unique bar code and may not be photocopied or downloaded.<\/p>\n<p><strong>The Procedure<\/strong><\/p>\n<p>1. Visit the Philippine Consulate in Vancouver or send a request letter along with a self-addressed prepaid return envelope to ask for a Fingerprint Card No. 5<\/p>\n<p>2. Fill in Form No. 5 with the required data on both sides of the card truthfully and legibly. In the case of a married woman, the order in writing the name is as follows:<\/p>\n<ul>\n<li>Maiden surname (father\u2019s surname)<\/li>\n<li>Husband\u2019s surname<\/li>\n<li>First name or given name<\/li>\n<li>Maternal surname (maiden surname of mother)<\/li>\n<\/ul>\n<p>3. Proceed to the Philippine Consulate General or to the nearest police agency (RCMP) for fingerprint impression (ROLLED IMPRESSION) on the appropriate spaces. The form (at the space provided) should bear the name, signature and official designation of the officer who did the fingerprinting.<\/p>\n<p>4. Attach a recent 2\u201d x 2\u201d photograph with white background (taken within three months prior to the application).<\/p>\n<p>5.
