A Filipino citizen is eligible to join this insurance system, allowing each member to save up for retirement or avail of the other benefits.<\/strong><\/p>\n<p><strong>First Time Registration<\/strong><br \/>\nTo become a member, an applicant needs to fill up an E-1 or Personal Record Form and submit required documents for verification purposes.<br \/>\nUpon completion of the form, submit it along with certified true copy and photocopy of any of the following: the following:<\/p>\n<ul>\n<li>Baptismal Certificate<\/li>\n<li>Driver&#8217;s license<\/li>\n<li>Passport<\/li>\n<li>Professional Regulation Commission (PRC) card<\/li>\n<li>Seafarer&#8217;s Identification and Record Book, or Seaman&#8217;s book<\/li>\n<\/ul>\n<p>If an applicant fails to secure any of the above documents, he or she can submit two of the documents listed below, each containing a recent photograph and date of applicant&#8217;s birth.<\/p>\n<ul>\n<li>Alien Certificate of Registration<\/li>\n<li>ATM card with cardholder&#8217;s name<\/li>\n<li>ATM card and, if it does not bear cardholder&#8217;s name, certification from the issuing bank that the account number belongs to the cardholder<\/li>\n<li>Bank Account Passbook<\/li>\n<li>Birth\/Baptismal Certificate of children.<\/li>\n<li>Certificate from the Office of the Northern\/Southern Cultural Communities or Office of Muslim Affairs<\/li>\n<li>Certificate of Naturalization issued by the Bureau of Immigration<\/li>\n<li>Company ID<\/li>\n<li>Company Representative Authorization Card issued by the SSS<\/li>\n<li>Credit Card<\/li>\n<li>Fishermen&#8217;s Card issued by the Bureau of Fisheries &amp; Aquatic Resources<\/li>\n<li>GSIS Card<\/li>\n<li>GSIS Certificate of Membership<\/li>\n<li>Health or Medical Card<\/li>\n<li>ID card issued by local governments (e.g. Barangay, Municipality, or City)<\/li>\n<li>ID card issued by professional associations recognized by PRC<\/li>\n<li>Life insurance policy<\/li>\n<li>Marriage contract<\/li>\n<li>NBI Clearance<\/li>\n<li>Overseas Workers Welfare Administration (OWWA) Card<\/li>\n<li>Pag-Ibig Member&#8217;s Data Form<\/li>\n<li>Permit to Carry Firearms issued by the Firearms &amp; Explosives Unit of the Philippine National Police<\/li>\n<li>PHIC Member&#8217;s Data Record<\/li>\n<li>Police Clearance<\/li>\n<li>Postal ID Card<\/li>\n<li>School ID<\/li>\n<li>Senior Citizen Card<\/li>\n<li>Tax Identification Card\/Affidavit<\/li>\n<li>Transcript of school records<\/li>\n<li>Voter&#8217;s Identification Card\/Affidavit<\/li>\n<\/ul>\n<p>Once you have an SSS Number, and have contributed into the system for one month, you may apply for an SSS ID card.<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-36355\" src=\"http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl.webp\" alt=\"\" width=\"520\" height=\"524\" srcset=\"http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl.webp 520w, http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl-298x300.webp 298w, http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl-150x151.webp 150w, http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl-417x420.webp 417w, http:\/\/www.pinoy-ofw.com\/pinas\/wp-content\/uploads\/sites\/10\/2015\/10\/sss_appl-300x302.webp 300w\" sizes=\"auto, (max-width: 520px) 100vw, 520px\" \/><\/p>\n<p><strong>SSS E-1 form (<a href=\"https:\/\/www.sss.gov.ph\/sss\/uploaded_images\/forms\/normal\/e1.pdf\" target=\"_blank\" rel=\"noopener\">download<\/a>)<\/strong><\/p>\n<p>Obtaining SSS ID<\/p>\n<p>To be able to obtain an SSS identification number, one must be an active member of the SSS with at least one month&#8217;s worth of contribution. Fill out an E-6 Form and submit the form at your local SSS office along with any of the following requirements:<\/p>\n<ul>\n<li>Passport<\/li>\n<li>Professional Regulation Commission (PRC) card<\/li>\n<li>Seaman&#8217;s book<\/li>\n<\/ul>\n<p>If the member cannot produce any of the three documents above, he or she can submit two of the following (with recent photograph):<\/p>\n<ul>\n<li>Driver&#8217;s license<\/li>\n<li>Valid National Bureau of Investigation clearance<\/li>\n<li>School or company ID<\/li>\n<li>Postal ID<\/li>\n<li>Senior citizen card<\/li>\n<li>Voter&#8217;s ID<\/li>\n<li>Savings account passbook<\/li>\n<li>Alien certificate of registration<\/li>\n<li>Government Service Insurance System member&#8217;s record<\/li>\n<li>Certification from the Office of Southern\/Northern Cultural Communities or Office of Muslim Affairs<\/li>\n<li>Taxpayer Identification Number (TIN) card<\/li>\n<li>It is recommended that submissions of copies of documents should be accompanied with original authentic document. SSS office will not keep the original copies but only use them for verification.<\/li>\n<\/ul>\n<p><strong>SSS E-6 form (<a href=\"http:\/\/pinoy-ofw.com\/guides\/sss-form-E6.pdf\">download<\/a>)<\/strong><\/p>\n<p><strong>ID Clearance and Photo<\/strong><\/p>\n<p>After submission of the E-6 form and accompanying documents, you will be granted an ID Clearance which is a small piece of paper where your scheduled ID capture will be printed. Depending on the workload of the office, you may be asked to return a few days later for ID capture.<\/p>\n<p>It is worth noting that during your scheduled ID capture, processing will be done on a first-come first-served arrangement. Once it is your turn, a clerk will attend to you and retrieve your personal data such as fingerprint, specimen signature, facial image and ask you to enter a confidential 4-digit pin on a keypad.<\/p>\n<p><strong>Issuance of SSS ID<\/strong><\/p>\n<p>After your turn at the SSS desk and ID capture, you&#8217;re basically done with your application. Next step is to wait for your SSS identification card and SSS ID by mail.
